voting buttons

I created a Message template which consists of customized form  which is the LEAVE form . This template also has voting buttons

 

Once I fill the Leave form and send to manager , Once the Manager receive the filled leave form email  and choose voting button as Approve  and choose either of the radio button Which is Send the response now or Edit the response before sending and click ok will pops up new email template . This email should have default  email id in CC  , How can I achieve this .
